My tiny contribution: I formulate "a lesson learnt" as something new valuable added to our knowledge and/or our way of doing things. It is usually different from hypothesis/expectations. However, not all lessons learnt can be recommendations. Once identified, it should be analyzed to decide if it would improve the intervention and whether it can be translated into a recommendation for other projects or other places.
